POTS, Posture, and the Patterns Weâre Missing
So close, but still missing the obviousđ¤Śââď¸.

Itâs been a little while since Iâve written here. Lifeâs been fullâgood things, busy things, the kind of movement that reminds you things are evolving.
Recently, in conversations with people after consultations, a question came up:
âWhat do you know about POTS?â
So I did what most people doâI looked it up.
Postural Orthostatic Tachycardia Syndrome.
And right there, in the name, you already have clues.
- Postural â structure
- Orthostatic â standing upright
- Tachycardia â increased heart rate
- Syndrome â we donât really know
That alone tells you quite a bit.
The Pattern Is Right There
When you dig into how POTS is described, itâs framed as a circulatory issue:
- Heart rate increases when going from sitting to standing
- Symptoms show up when upright
- Blood pooling below the heart
- Lightheadedness, fatigue, brain fog
And yet, whatâs the common thread?
Position. Posture. Structure.
From a terrain perspective, thatâs not a small detailâthatâs the starting point.
The Problem With âSyndromesâ
When something is labeled a syndrome, it usually means:
âWe see a pattern⌠but we donât understand the mechanism.â
So what happens?
You get a list of symptoms:
- Fatigue
- Brain fog
- Headaches
- Palpitations
- Nausea
- Exercise intolerance
And the reality isâŚ
Almost anyone could qualify at some point.
Heat, stress, dehydration, poor sleepâthese all shift the body. The body is dynamic. It adapts constantly.
Stand up quickly on a hot day?
Your heart rate changes.
Thatâs not mysteriousâthatâs responsiveness.
What If Itâs Structural First?
If symptoms are changing based on positionâŚ
Then we have to ask:
What is happening structurally when the body changes position?
Because structure dictates flow.
- Blood flow
- Lymph flow
- Nerve signaling
- Pressure gradients
If the body is misaligned, compressed, or pulled by adhesions and scarsâŚ
You donât get clean flow.
You get compensation.
Think About This Simply
If your body were a system of tubes and pumps (which it is), and you:
- Twist it
- Compress it
- Pull on it
What happens?
Flow changes.
Pressure changes.
Output changes.
Now ask:
Would that affect heart rate? Blood pressure? Energy?
Of course it would.
The Missing Piece: Internal Strain Patterns
This is where most conversations stop short.
Because itâs not just âpostureâ like standing up straight.
Itâs:
- Scar tissue pulling internally
- Adhesions restricting movement
- Old injuries creating asymmetry
- Surgical history altering tension patterns
These are traction lines inside the body.
And they donât just affect musclesâthey affect everything downstream.
The Osteopathic Lens
Before chasing symptomsâŚ
Before stacking protocolsâŚ
Before labeling conditionsâŚ
You have to ask:
Is the body free to move the way itâs designed to?
That means:
- Releasing restrictions
- Reducing internal pulling
- Restoring alignment
- Opening pathways of elimination
Not forcefully.
Not aggressively.
But gently and intelligently.
Why the Heart Rate Changes
If blood is poolingâŚ
If vessels arenât responding efficientlyâŚ
If the system is under strainâŚ
Then the body adapts.
It doesnât âmalfunction.â
It compensates.
So the question shifts from:
âWhy is the heart rate abnormal?â
To:
âWhat is the body adapting to?â
One Diagnosis⌠or Many?
Hereâs where it gets interesting.
POTS is often mentioned alongside:
- Fibromyalgia
- IBS
- Fatigue conditions
- Anxiety patterns
So whatâs going on?
Different labelsâŚ
Same terrain.
Same underlying stress patterns.
Same structural and energetic disruptions.
The Bigger Picture
From a terrain perspective, youâre not chasing diagnoses.
Youâre looking for patterns:
- Structural
- Energetic
- Emotional
- Biochemical
And asking:
Where is the interference?
Because once you remove interferenceâŚ
The body reorganizes.
What Iâve Seen
Have I seen people with a POTS diagnosis improve or no longer carry that label after working structurally?
Yes.
Does that mean every case is purely structural?
No.
But it means this piece is often overlookedâand it shouldnât be.
Start With the Clues
My dad always approached this like a detective.
Not chasing names.
Looking at clues.
And in this case, the clues are right in the name:
Posture. Position. Response.
So the better question becomes:
- What injuries has this person had?
- What scars are present?
- What internal strain patterns exist?
- Has the structure ever truly been addressed?
Final Thought
You donât need to accept a label as the final answer.
You can use it as a starting point.
Because when you look through the terrain lensâŚ
A lot of âmysteriesâ stop being mysterious.
They start making sense.
